Who are you serving with

your DSMES service?

You need to know

your population's demographics,

like ethnicity,

sex,

age,

levels of education, and

prevalence of diabetes.

The information can be found in

published sources by the US Census,

CDC, or your state health department.

You also need to know what

your audience's needs are.

Needs might include new resources

like interpreter services

or multi-language

or large-print materials.

Or they might include

changes to existing services,

like shifting meeting hours,

adding more meeting locations,

or offering services

through mobile apps or telehealth.

Through evaluation,

you'll be better prepared to reach

your population by knowing

their collective needs.
